Terms of Service — Patient Feedback Survey

Effective date: 01 August, 2025

These Terms of Service (the “Terms”) govern your use of the Sidra Kuwait Hospital OutPatient feedback survey (the “Survey”). By accessing or submitting the Survey, you agree to these Terms.

If you do not agree, do not use the Survey.

1) About this Survey

  • The Survey collects feedback to help us improve our services and patient experience.
  • The Survey is optional and separate from your clinical care. Participating (or not) will not affect your access to care.

2) No medical advice & not for emergencies

  • The Survey is not a channel for medical advice, diagnosis, treatment, or urgent communication.
  • If you need medical help, contact our call-center to book your appointments directly at 1830000.
  • For emergencies in Kuwait, call 112 immediately.

3) Eligibility

  • You may complete the Survey for yourself.
  • If you are under 18, a parent or legal guardian should complete it or consent to your participation.
  • If you are completing the Survey on behalf of another person (e.g., your child or a dependent), you confirm you are authorized to do so.

4) Privacy

  • Our handling of personal data is described in our Privacy Policy (linked on the Survey page). Please review it before you submit responses.
  • We minimize the personal data we collect and use it only for the purposes described (e.g., service quality, safety, and compliance). Where legally required, we may also use de‑identified and aggregated data for reporting and improvement.

5) Your responsibilities

You agree that you will not:

  • Provide false, misleading, or unlawful information.
  • Attempt to disrupt the Survey or interfere with its security or availability.
  • Upload or share content that infringes others’ rights, or contains malware or harmful code.

6) Ownership & permitted use of responses

  • You own your responses. By submitting the Survey, you grant Sidra Kuwait Hospital a non‑exclusive, worldwide, royalty‑free license to use, store, reproduce, and create derivative works from your responses solely to operate, improve, evaluate, and report on our services and patient experience.
  • We may use de‑identified and aggregated responses for quality improvement, education, and research, consistent with applicable law and our Privacy Policy.

7) Data retention & deletion

  • We retain Survey data for as long as needed for the purposes described above and to meet legal or regulatory obligations.
  • Where applicable, you may request access, correction, or deletion of your Survey data by emailing info@sidrakwhospital.com. We will respond as required by law.

8) Intellectual property

  • The Survey’s content, design, and technology are owned by Sidra Kuwait Hospital or our licensors and are protected by law. You may not copy, modify, or distribute them except as allowed by these Terms.

9) Changes to the Survey or these Terms

  • We may update the Survey or these Terms from time to time. When we do, we will update the “Effective date” above and, where required, provide additional notice. Your continued use after updates means you accept the revised Terms.

10) Disclaimers

  • The Survey is provided on an “as‑is” and “as‑available” basis. To the fullest extent permitted by law, we make no warranties, express or implied, including merchantability, fitness for a particular purpose, or non‑infringement.

11) Limitation of liability

  • To the maximum extent permitted by law, Sidra Kuwait Hospital will not be liable for indirect, incidental, special, consequential, or punitive damages, or any loss of data, arising from or related to your use of the Survey.

12) Governing law & venue

  • These Terms are governed by the laws of the State of Kuwait. Courts located in Kuwait shall have exclusive jurisdiction over disputes arising from or related to the Survey or these Terms.
